Master of Social Work/ MSW – Acute Care

Job Summary: An Acute Care Facility in San Francisco, CA is looking for a Master of Social Work/MSW to join their team for a 13-week travel assignment. Individuals recovering from illness or trauma may become psychologically, emotionally, and socially destabilized; it is the role of a Preferred Healthcare Staffing medical social worker to provide support for these patients.

• Identify any special medical and education needs the patient may require
• Help Create a plan with the patient and other members of their health care team in order to developing an effective care plan
• Facilitate care coordination and communication among all involved parties
• Provide support for the patient and family/caregivers to identified appropriate care coordination is received

Requirements:
• Current BLS/CPR Certification
• Master’s in Social Work, Counseling, or related field
• 2 years of experience within the last 4 years as an MSW
• Experience in care coordination, discharge planning or case management in a medical setting
