The following are a few tips that will help to get you on your way:
GET AND MAINTAIN A SHOT CARD
As trends for ongoing vaccinations increase, Preferred Healthcare encourages you to get a shot card and track them. If you do not have a shot card, see your doctor and find out how to get one. A sample shot card is included in the helpful links section of this email. Health Histories can be checked and verified so shot cards can be re-written by your medical provider to a certain extent, allowing you to have one on hand.
TB CLEARANCE
All healthcare professionals must have appropriate TB clearance in their file. TB clearance is obtained by having one of the following:
- A current TB PPD skin test result within the last 12 months. *please note, that some facilities may require completion of 2-step PPD skin testing.
- IF you have a history of testing positive to the PPD skin test or are allergic to the PPD skin test, TB clearance is obtained by having a: Chest X-ray (normal) within the last 4 years-This must be accompanied by an annual TB questionnaire.
- Quantiferon (Lab test) This test is an in vitro diagnostic aid that measures a component of cell-mediated immune reactivity to M. tuberculosis.
REQUIRED VACCINATIONS
More and more facilities are also updating the vaccination requirements to include the completion of serologic evidence of immunity (titers). In some cases, despite having been vaccinated or even having a history of the disease, you may not be immune! A titer will help evaluate your immunity and determine if further vaccination or boosters may be required.
The primary vaccinations facilities are updating requirements for are:
- Varicella (chickenpox) 2 shots – In some cases, having proof of a history of varicella (chickenpox) as documented by your medical provider may be acceptable, however, facilities are updating their requirements to be vaccination and/or titer.
- MMR (mumps, Rubeola, Rubella) 2 shots — provide proof of vaccination or a titer showing immunity for each
- Hepatitis B. 3 shots — vaccinations series are recommended with a follow-up titer to show immunity, however, some of our facilities are still accepting a declination
- Flu – provide proof of vaccination or waiver. If waiver, client may require a mask be worn during all shifts.
With the growing concerns for communicable diseases…this proactive step helps ensure the safety of both you and the patients you treat.

Using the Online System
For your convenience the majority of your credentialing items can now be completed through our online system. This will allow you access to your dashboard for a quick glance at what you are lacking and what needs to be completed.
No longer will you have to print and fax back piles of paper work. Simply skip the online application portion and log in by clicking the “my home page” button at the top right. You will log in with your username and password that will be sent to you in a separate email.
